Covid 19 Screening Questions
As the recommendations and thoughts on our safety and health as a nation keep changing and sometimes it's unclear as to whether you should be visiting the practice. We want you to be aware of the questions we are asking all of our patients before appointments. These are the five questions we will ask
1)Have you tested positive for Covid 19 in the last 7 days?
2)Are you waiting for a Covid 19 test or the results?
3)Do you have any of the following symptoms – new continuous cough, a high temperature/fever, a change or loss of smell/taste?
4) Do you live with someone who has either tested positive for Covid 19 or have had symptoms of Covid 19 in the last 14 days?
5) Have you been notified by NHS test and trace in the last 14 days that you are a contact of a person who has tested positivie for Covid 19 and you do not live with that person?
If you have answered YES to any of these questions then we do not want to see you are the surgery at this time and please contact us to rearrange any scheduled appointments.
If you have recovered from Covid 19 or completed a period of isolation that is alright. Some symptoms can persist beyond the 14 days. If you are unsure please contact the surgery anyway on 01904 781987.
